: Although the study had a dual purpose, as quality improvement and research, when submitted to the University Behavioral Research Ethics Board it was exempted as program evaluation/quality improvement. To use the data for research purposes we subsequently requested review for secondary use of data for research, which was approved. Regarding consent, where team members deemed the patient cognitively capable of participating in the research, they introduced the study and provided materials at the end of the case conference (consent form, mail-in questionnaire, pre-stamped envelope). With potential participants’ permission, the team provided the phone number of those interested in being contacted for an interview to the researchers. Informed consent was obtained from all study participants. During the research interview the consent form was reviewed aloud and participants’ verbal consent was included in the audio-recording. Completion of the questionnaire constituted implied consent. The study adhered to the Declaration of Helinski principles for research involving human participants.
