: Although this project was not research, but rather quality improvement related to vaccine conversations between patients and HCPs, we conducted the study in accordance with the Declaration of Helsinki. The study was screened through A pRoject Ethics Community Consensus Initiative program (ARECCI) [ ]. This is the approved process for such projects which identifies any ethical risks that may be present and in need of addressing prior to having patients or others participate in quality improvement or evaluation projects in health and human services. All participants involved in the working group, survey and discussion groups were informed of the study, its purpose, use of any data gathered, as well as their involvement as participants. They were asked to consent in writing and again verbally before the start of activities that they understood the study and related activities they would be involved with, as well as how any data gathered during those activities would be used. The proposed plan involving patients/families and care providers in discussions was screened November 23, 2023, and the survey development and draft used for testing was screened January 12, 2024. The resulting risk score for both screenings was ‘5’ which is ‘minimal risk’. No further ethics screening was required.
