: The study has been conducted in adherence to the Declaration of Helsinki and was approved by Swedish Ethical Review Authority (Gothenburg office) (2020–05572). The need of informed consent was waived by the Swedish Ethical Review Authority. All analyses were conducted on a large amount of pre-existing registry data from the patients’ EMRs. This is also in accordance with the Declaration of Helsinki, that states: “Where consent is impossible or impracticable to obtain, secondary research on stored data or biological material may be done only after consideration and approval of a research ethics committee” (paragraph 32).
