: All patients suffering from multimorbidity signed an informed consent to participate. All methods were performed in accordance with the relevant guidelines and regulations (Declaration of Helsinki). The local ethics committee “Comitato Etico Regione Toscana - Area Vasta Nord Ovest (CEAVNO)” waived the need of ethical approval for the study. To record the focus group with professionals, we obtained their verbal authorization.
