Ethics approval and consent to participate: The study was conducted following the principles of the Declaration of Helsinki. Under the Norwegian Personal Data Act, fully anonymous data are not personal data, therefore data protection consent is not required. We collected only anonymous data, thus after consulting with the Norwegian Data Management Service (SIKT), we concluded that a signed consent form was not required. According to the Norwegian Health Research Act, quality assurance or health service evaluation projects fall outside the scope of the Act and do not require approval from the Regional Committees for Medical and Health Research Ethics (REK).
