Ethics approval and consent to participate: Ethics approval for this study was received from The Alfred Ethics Committee (127/22) and from the NSW Population & Health Services Research Ethics Committee (2022_ETH02415). The study was conducted in accordance with the Declaration of Helsinki and the Australian National Statement on Ethical Conduct in Human Research. As this study is a retrospective analysis of a cohort of people who are deceased, informed consent could not be obtained. Both ethics committees approved a waiver of consent under Sect. 2.3.10 of the Australian National Statement on Ethical Conduct in Human Research.
