Ethics approval and consent to participate: Ethics approval for this study was received from The Alfred Ethics Committee (127/22) and from the NSW Population & Health Services Research Ethics Committee (2022_ETH02415). As this study is a retrospective analysis of a cohort of people who are deceased, informed consent could not be obtained.
