Ethics approval and consent to participate
: Ethical review for the Malawi study was obtained from the Malawi National Health Sciences Research Committee (NHSRC; protocol #1072) and London School of Hygiene and Tropical Medicine (LSHTM) Ethics Committee (protocol #6303). Participants provided written informed consent for interviewer-led questionnaires, anthropometry and fasting blood samples. Ethical approval for the ALSPAC cohort was obtained from the ALSPAC Law and Ethics Committee and the Local Research Ethics Committee. Parents provided written informed consent regarding their own participation. The main caregiver initially provided consent for child participation, and from the age 16 years, the offspring themselves have provided informed written consent.
