: No formal approval was sought from the ethics committee as the manuscript was the outcome of an expert consultation process. The standard protocols of the International Agency for Research on Cancer/World Health Organization (IARC/WHO) for expert consultation process were followed. An official invitation letter signed by the IARC Director (Elisabete Weiderpass) and Partha Basu was sent to each expert seeking their consent to participate. Each of the experts participating in the consultation and acknowledged in the manuscript provided written consent to participate and signed a declaration of conflict-of-interest that was reviewed and approved by the Secretary of the ethics committee at IARC.
