: This study was performed in line with the principles of the Declaration of Helsinki. The study and the data linkages have been approved by The Regional Committee for Medical and Health Research Ethics (REC South East A, reference number 15538), the Norwegian Institute of Public Health, the Norwegian Directorate of Health (Norwegian Patient Registry), and Statistics Norway. Data were handled according to the General Data Protection Regulation, Article 6.1.e “[The] processing is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller”. A Data Protection Impact Assessment has been performed in consultation with the Data Protection Officer in the Norwegian Institute of Public Health. The study is based on national health registries to which reporting is mandatory.
