The study was approved by the Medical Ethical Committee of the Amsterdam UMC and registered in the Dutch Trial Register (6531). Passive written informed consent for participation was obtained from the caregivers of all of the adolescents. In addition, active oral informed consent (in relation to cash receipts) and written informed consent (in relation to the questionnaires) to participate were given by all adolescents.
