This nationwide retrospective cohort study was approved by the Ethical Review Board in Lund (number 2012/795 and later amendments). All the individuals involved in this study had pseudonymized serial numbers in the database to protect their integrity. Patient consent was not required for this study. The database is located at the Center for Primary Health Care Research at Lund University in Malmö, Sweden.
