: All human subjects gave written informed consent under protocols approved by either the Oregon Health and Science University (OHSU) Institutional Review Board (approval # IRB10543) and the Michigan State University Twin Registry (MSUTR IRB). For subjects under 16 years old, written informed consent was obtained from the parents or legal guardians. Additionally, for subjects under age 16, written informed assent of the subject was obtained, and for subjects over age 16, written informed consent was obtained.
