: The study was conducted according to the Declaration of Helsinki. The institutional review boards of Cincinnati Children’s Hospital Medical Center and all delivery hospitals approved this study. Brown University and the University of Massachusetts Amherst deferred to the CCHMC IRB as the IRB of record (protocol #2015–6165). Caregivers provided written informed consent for themselves and their children after study protocols were explained and their questions answered. Adolescents provided written informed assent at the age 12-year visit.
