: The regional Medical Ethical Committee in Umeå approved the study (Dnr-05-130 M). The patients and voluntary controls were informed and gave their written consent to participate. All muscle samples were collected in agreement with the Declaration of Helsinki. Autopsy specimens were collected in agreement with Swedish laws and regulations on autopsy and transplantation.
