Ethics approval and consent to participate
: The final protocols and informed consent forms were approved by the local ethics committee (e-Appendix ). The studies were performed in accordance with the Declaration of Helsinki, the International Council for Harmonisation/Good Clinical Practice guidelines, applicable regulatory requirements and the AstraZeneca policy on Bioethics.
