Ethics approval and consent to participate: Prior to 2019, all patients provided written, informed consent. To ensure a representative population, a transition to opt-out consent was approved by the Australian Commission for Safety and Quality in Health Care in 2019. ASAR is under the governance of the Thoracic Society of Australia and New Zealand (TSANZ). Recruitment commenced in 2013, with systematic, de-identified, annual data collection via an electronic data capture system (REDCap) [15]. Ethical approval was obtained according to each site’s Human Research Ethics Committee in Australia (2019/ETH03836) and New Zealand (2/CEN/69/AM09).
