: All procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional and/or national research committee and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. Ethical approval for all study procedures was obtained from the Institutional Review Board at Harvard University, Yale University, Mbarara University of Science and Technology, and the Uganda National Council for Science and Technology and the Research Secretariat in the Office of the President in Uganda.
