Ethics approval and consent to participate
: This study was approved by the Kenya Medical Research Institute (KEMRI) Scientific and Ethics Review Unit (SSC 2772). Permission to conduct the research was also obtained from the County Departments of Health. Prior to fieldwork, meetings were held with health facility managers and health workers to explain to them the nature and purpose of the study. Verbal consent was obtained for observations and written consent for all formal interviews.
