: The health facility surveillance did not impose any changes in the national treatment guidelines and data used in the analysis were gathered as part of routine care. Consent was waived by the ethics committee; therefore, individual patient consent was not sought. All the records were pseudo-anonymized at the point of data capture in the healthcare facilities but linked to the demographic surveillance by an ID number. This study was approved by the Kenya Medical Research Institute Scientific Ethics Review Unit (KEMRI/SERU/CGMR-C/106/3592) and the Oxford tropical research ethics committee (OxTREC Reference: 511-18).
