The caregivers of study participants aged < 20 years provided informed consent (while participants provided assent). Study documents were approved by the Burkina Faso Ministry of Health Research Ethics Committee, CNRFP Institutional Bioethics Committee, the London School of Hygiene and Tropical Medicine ethical Committee and Durham University Department of Biosciences Ethics Committee.
