: The protocol for this study was reviewed and approved by the Center for Global Health at the Centers for Disease Control and Prevention in Atlanta, GA (Protocol # 7230.0) as well as by the Madagascar National Ethics Committee (095-MNSANP/SG/-AGMED/CNPV/CERBM). An adult member of the household provided written informed consent for the household interview. Additional consent of a parent/guardian was obtained for acquisition of capillary blood samples from participants aged 2 months to 14 years; assent was also obtained from those 7–14 years of age.
