: Ethical approval for the study was obtained from the London School of Hygiene and Tropical Medicine (LSHTM; ref#13600) and Arba Minch University (AMU; ref#5991/111) and all study procedures were performed in accordance with relevant guidelines and regulations. After receiving ethical clearance, written permission was obtained from the Arba Minch Zuria District Health Office.
