: The trial and the mixed-methods study were conducted according to the principles stated in the Declaration of Helsinki (Ethical Principles for Medical Research Involving Human Subjects) as amended in 2008, all applicable regulations and according to established international scientific standards. The study protocol was reviewed and approved by the ethical committees of the Faculty of Medicine Gadjah Mada University in Indonesia (KE/FK/522/EC/2016), the Human Research Ethics Committee (HREC) of the Northern Territory (NT) Department of Health and Menzies School of Health Research in Australia (15-2517), and the Institute of Tropical Medicine in Belgium (1163/17). For the qualitative strand, oral consent was collected from all participants and, for minors, from a legal guardian. For the quantitative survey, written informed consent was obtained from all participants or legal guardians in case they were underage. Illiterate participants willing to participate provided consent by a fingerprint in the presence of a witness.
