: Use of aggregate monthly consultation counts from PHC registers for this study was authorized by the Madagascar National Ethics Committee and the Medical Inspector of Ifanadiana district. It was deemed non-human subjects research by Harvard University’s Institutional Review Board. The IHOPE longitudinal survey implemented informed consent procedures approved by the Madagascar National Ethics Committee and the Madagascar Institute of Statistics. This included obtaining informed consent from all subjects or their legal guardians. Household-level de-identified data from the IHOPE survey were provided to the authors for the current study.
