Ethics approval and consent to participate: Ethical clearance (Ref. 139/2016) was obtained from the Institutional Review Board (IRB) of the Salale University, College of Medicine and Health Science Department of Public Health. The letter of permission was secured from the administrative bodies of the Kellem Wollega Zone Health Office and Hawa Gelan district. Written informed consent was obtained from each study participant. All of the study participants were verbally informed about the purpose of the study, and their right to participate, or withdrawal from the study at any time. Privacy and confidentiality were strictly maintained by avoiding writing the name of the study participants during the data collection, and all methods were carried out in accordance with relevant guidelines and regulations.
