Ethics approval and consent to participate
: The study was approved by the Regional Committees for Medical and Health Research Ethics West (REK 2015/879) and is registered in(NCT02610959). The study was conducted and performed according to the Declaration of Helsinki. Participation in the study was voluntary and written informed consent was obtained from the participants after giving both written and oral information about the study. The participants could withdraw from the study at any time without giving any reason and this was highlighted in the declaration of informed consent.
