Ethics approval and consent to participate
: We obtained ethical approval for this study from the University of Victoria, the University of British Columbia, and from each of the five regional health authorities in British Columbia that were partners in the ELPH project: UVic/Vancouver Health Authority file J2011–102, UB/Vancouver Coastal Health Authority file H11–03359, Fraser Health Authority file FHREB2012–003, Interior Health Authority file 2011–12-044-E, and Northern Health Authority file RRC-2012-0006. We obtained consent from all participants prior to participating in the study.
