Ethics approval and consent to participate
: The qualitative section of this study is part of the M.S.c thesis was approved by the research council of Tehran University of Medical Sciences(Approval No:86400023). At the time the proposal of the study was sent to the University, there was no separate ethics committee, the study was approved to be conducted by the Research Council, after taking into account all issues, including ethical considerations. The literature review and documentary review has no need to ethical approval. Additional permissions and written consent were also acquired from the research participants prior to each interview. We assured participants that all collected information would be treated confidentially.
