Ethics approval and consent to participate

: As this was a secondary data analysis, there was no need for consent for ethics approval. The SABE study was approved by both the Institutional Human Ethics Committee of University of Valle (Colombia) and the Bioethics committee of University of Caldas (Colombia) (Records number 09–014, O11–015, and code CBCS-021-14, respectively). All survey respondents gave informed consent before participation and all information was collected confidentially.
