: Informed consent was obtained before initiating the interviews and/or participant observation studies following approval by the Institutional Review Board of Ohio University (17-X-173) and Pontificia Universidad Católica del Ecuador. Consent forms were written in Spanish and administered to participants. Participants were then asked to sign and return one form for our records and were given a separate form to keep for themselves.
