Ethics approval was obtained from the Institute Ethics Committee of All India Institute of Medical Sciences (AIIMS), New Delhi, all collaborating medical institutions, and the University of Southern California (UP-15-00684-AM020). The Health Ministry Screening Committee of the Indian Council of Medical Research provided administrative clearance to the LASI-DAD research study (2202–16741/F1). A written informed consent form was obtained from each respondent in the presence of a family member. A consent form was translated into the participant’s local language and consent was obtained with the participant’s signature or a thumbprint.
