: Ethical approval was granted for studies using NethOSS data by the Institutional Review Board of University Medical Centre Utrecht (reference number 14-127). According to the conditions of the Medical Ethics Review Committee (METC) this study falls within the scope of non-WMO research and does not need to be authorised by the METC to proceed. All participants of the interviews were required to sign an informed consent form prior to the interview. At the beginning of the interview, participants were made aware of the nature of the study and reminded of their voluntary participation in the study in addition to their right to withdraw participation at any time during the interview.
