: Ethical approval for this study was obtained from the ethics committees of the London School of Hygiene and Tropical Medicine, London, UK and the Centre for the Rehabilitation of the Paralysed (CRP), Dhaka, Bangladesh. Either written or verbal informed consent was obtained from the respondents.
