Ethics approval and consent to participate
: This study was ethically approved by Tehran University of Medical Sciences, and by health and political administrative bodies of Abbottabad District, Pakistan. A detailed description of the study, benefits, confidentiality and the informed consent procedures was explained during the initial contact with the prospective participant prior to their participation. The informed consent form was signed before the start of an initial interview. All information gathered in this study would be treated as confidential and all the information about the participants could only be accessed by the research team and their real identities were not used in any verbal or written presentation/reports.Participants were informed that they can withdraw from the interview and have the right to decline to answer any question at any time.
