: SAK is an associate professor in Biostatistics based in the Department of Nutrition and Food Sciences at the American University of Beirut. SAK’s research is in the theory and applications of Bayesian statistics. His main area of research and consulting activity is in the theory of inference, computational aspects of Bayesian statistics and in Bayesian modelling generally. He has been involved in many application areas, particularly in medicine and Health Economics.

: Although the article contains studies with human participants, the author hasn’t got involved in the valuation study at all. The author has just used the data to illustrate the models of interest. The author has the permission from Prof John Brazier, who was investigator in the original SF-6D valuation survey, to reuse the data. However, if people want to use it, then they need to contact Professor John Brazier at the University of Sheffield, UK.A key point worth mentioning is that the original study was conducted in 1999 and that the University of Sheffield did not require ethics committee approval for the study at that time-such a thing did not exist in 1999 (personal communication with Prof John Brazier (j.e.brazier@sheffield.ac.uk)).
