: The DenHeart study complies with the Declaration of Helsinki. Danish legislation does not require surveys to be approved by an ethics committee system but rather by the Danish Data Protection Agency (2007-58-0015/30–0937). The Danish National Board of Health permitted the use of register data. DenHeart is registered at ExternalRef removed (NCT01926145). Patients provided informed consent.
