Ethics approval and consent to participate
: The study protocol conforms to the ethical guidelines of the Declaration of Helsinki as reflected in a prior approval by the Institution's human research committee of Algarve University Hospital Center (22/07/2016). Patients signed an informed consent form prior to inclusion in the study. All data was collected anonymously, and no association could be established between the questionnaires and the responders.
