: The study obtained ethical approval (NWU-00091-16-A1) from the Health Research Ethics Committee of the North-West University and is registered at (NCT04056377). This study conformed to the revised Declaration of Helsinki and followed the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) guidelines. All assessments and procedures were performed according to the Guidelines for Good Clinical Practice.
