Ethics approval and consent to participate
: This study was approved by the Liverpool School of Tropical Medicine, UK, Makerere University School of Public Health, Uganda and the ethics committee at Lacor Hospital. The study was registered with Uganda National Council of Science and Technology (UNCST). Interviewed respondents first gave their written consent before participating in the study. Data access was restricted to the researchers and only used for research purposes.
