: This study was approved by the University of Warwick’s Biomedical and Scientific Research Ethics Committee (REGO-2017-2099), the Ghana Heath Service Ethics Review Committee (GHS-ERC-010/09/17), the Malawian National Health Sciences Research Committee (17/09/1904) and the Partners Healthcare Institutional Review Board in the USA (2018P000790).
