: Ethical approval for the study was obtained from the Liverpool School of Tropical Medicine Research Ethics Committee (17-044), the Sierra Leone Ethics and Scientific Review Committee, Ministry of Health and Sanitation, the DRC Ministry of Research Multidisciplinary Centre for Development Bunia (024/2017) and University of Liberia Pacific Institution for Research and Evaluation Review Board (16-09-009).
