Ethics approval and consent to participate

: All data were transferred anonymously into the database. Case files were collected month for month from the archives and kept in the office of the researcher during the data collection process. Name of involved health worker was not noted, only cadre. The study was approved as part of a larger study project by the National Health Science Research Committee of Malawi (approval number 1995) and fully endorsed by hospital management.
