: Ethical approval was obtained from the Scientific and Ethics Committee of Swaziland and the Health Sciences Research Ethics Committee (IRB00006240), University of Free State. Authorisation of the study was granted by the NTCP and MSF. All eligible participants were given necessary information about the study.
