: EW and RM are employed by the University of Southampton to work for NETSCC. NETSCC runs the NJL for the NIHR. EW receives salary from the NIHR for her role as Director of Research Delivery and Impact at NETSCC, which includes management of the NJL. RM was employed by NETSCC during the conduct of the study and over the last 20 years has worked at various times for the NJL and its publishing predecessors. AlM, KS and TW have received fees or a proportion of their salary from the NIHR to support NJL activity. AlM is Chair of the NJL Editorial Group (Health Services and Delivery Research, Programme Grants for Applied Research and Public Health Research journals) and is an editor for the NJL. KS is Chair of the Health Technology Assessment Monograph Editorial Board and is a member of the NJL board. TW is Editor in Chief for the NJL. CB and DW have received payment from NETSCC to support the development and delivery of the relaunched NJL, and preparations of this paper.
