: The scientific and ethical aspects of the study were reviewed and approved by the Institutional Scientific Research Committee of the Health Strategy and Policy Institute, Hanoi. Ethical approval was also obtained from the WHO Research Ethics Review Committee (Protocol No. ERC.0003085).
