: Ethical issues in this project relate to confidentiality of personal information and potential respondent burden. For all participants, we will follow standard procedures to ensure informed consent to participate through provision of detailed, accessible information, opportunity to consider and receive answers to questions and assurance of no care implications of nonparticipation. During group data generation, complete confidentiality cannot be assured, but all participants will be requested not to divulge information. Ethics approval has been obtained from the School of Health & Related Research, University of Sheffield, the National Bioethics Board, Pakistan, and the Health Research Ethics Board-Health, University of Alberta.
