Each participant received a plain language statement and opportunity to ask questions about the study before completing the survey, and when applicable, commencing the interview. Informed consent was deemed given with the voluntary completion of the survey. Each interviewee had volunteered to be interviewed, with verbal consent recorded at the beginning of the interview.
