: All research was conducted in accordance with the Declaration of Helsinki. University ethical approval for the first wave of interviews was secured from University College London Research Ethics Committee in September 2018 (UCL Ethics ID: 1037/003). Approval for the second and third waves of interviews was secured from University College London Research Ethics Committee in 2021 (UCL Ethics ID: 2037/004), which was later formally amended to include retrospective documentary analysis and additional interview questions in July 2023. All participants provided informed verbal and written consent prior to participating.
