: This paper was drafted from a large study that employed a mixed methodology approved by the University of Toronto Office of Research Ethics #43974 and the Uganda National Council of Science and Technology #HS3050ES. The Ministry of Health (MoH) issued introductory letters to districts and participating institutions. Formal approval was sought from the Chief Administrative Officer of each district. Participants provided written consent before the interview.
