: We contacted participants by phone or email for the initial introduction of the interviewer and research topic. We emailed the study information sheet in advance to allow time for informed consent. All participants consented to be interviewed, audio recorded and quoted in research dissemination. The interviews were conducted in a private room (both physical and virtual) with just the interviewer and participant. All study participants were financially compensated for their time. The study received ethical approval from the College of Medicine Research and Ethics Committee (COMREC) in Malawi, ref. P.11/21/3462, and the Liverpool School of Tropical Medicine in England, ref. 21–086.
